Trial Class Safety & Liability Waiver

Our mission is to provide a safe, respectful, family-oriented, and professional environment where every student can learn, grow, and enjoy Brazilian Jiu-Jitsu with confidence.

Before participating in any trial class or academy activity, please carefully read the following information.

I understand that Brazilian Jiu-Jitsu is a contact sport involving physical activity, including but not limited to takedowns, throws, grappling, joint locks, choke techniques, ground fighting, conditioning exercises, and close physical contact. I acknowledge that participation in these activities involves inherent risks that cannot be completely eliminated, including but not limited to cuts, bruises, sprains, strains, muscle injuries, joint injuries, fractures, concussions, serious bodily injury, permanent disability, property damage, and, in extremely rare circumstances, death.

I voluntarily choose to participate in the trial class and fully accept and assume all ordinary risks naturally associated with Brazilian Jiu-Jitsu and related training activities. I understand that my participation is voluntary and that I may stop training at any time if I feel unable or unwilling to continue.

I certify that I am physically and mentally able to participate in Brazilian Jiu-Jitsu training. I confirm that I have no medical condition, injury, illness, or physical limitation that would make participation unsafe unless I have fully disclosed such condition to the instructors before participating. I agree to immediately inform an instructor if I experience pain, dizziness, illness, injury, or any condition that may affect my safety or the safety of others during training.

I agree to follow all instructions provided by the instructors and staff of DJTEAM Brazilian Jiu-Jitsu Academy. I understand the importance of training safely, respecting my instructors and training partners, tapping immediately when necessary, and immediately releasing any submission or technique when my training partner taps or when instructed to do so.

I agree to maintain proper personal hygiene by wearing clean training clothing, keeping my fingernails and toenails properly trimmed, maintaining good personal hygiene, and refraining from participating in class if I have any contagious illness, contagious skin infection, open wound, or any other condition that could place myself or others at unnecessary risk.

In the event of an accident or medical emergency occurring during my participation, I authorize DJTEAM Brazilian Jiu-Jitsu Academy and its representatives to obtain emergency medical treatment on my behalf if I am unable to do so myself. I understand and agree that I am solely responsible for any medical expenses, transportation costs, hospital charges, or other healthcare expenses resulting from such treatment.

To the fullest extent permitted by applicable law, I voluntarily assume the ordinary risks inherent in participating in Brazilian Jiu-Jitsu and release, waive, and discharge DJTEAM Brazilian Jiu-Jitsu Academy, its owners, officers, instructors, coaches, employees, assistants, volunteers, independent contractors, affiliates, landlords, and property owners from claims arising from those inherent risks while participating in academy activities or using academy facilities, except where liability cannot legally be waived under applicable law.

I understand that personal belongings brought onto the academy premises remain my sole responsibility. DJTEAM Brazilian Jiu-Jitsu Academy shall not be responsible for the loss, theft, or damage of personal property.

I understand that photographs and video recordings may occasionally be taken during classes for instructional, educational, promotional, advertising, website, or social media purposes. Unless I notify DJTEAM Brazilian Jiu-Jitsu Academy in writing prior to participating, I authorize the academy to use my name, likeness, photographs, and video recordings for these purposes without compensation.

If the participant is under eighteen (18) years of age, this Trial Class Safety & Liability Waiver must be reviewed and accepted by the participant's parent or legal guardian. By submitting this form on behalf of a minor, the parent or legal guardian certifies that they are the legal parent or guardian of the participant, have the legal authority to provide consent, have carefully read and understood this entire document, voluntarily authorize the minor's participation in the trial class, acknowledge and accept the inherent risks associated with Brazilian Jiu-Jitsu, and agree to all terms and conditions contained in this Trial Class Safety & Liability Waiver on behalf of the minor.

By checking the box below, I acknowledge that I have carefully read this Trial Class Safety & Liability Waiver, fully understand its contents, voluntarily agree to all of its terms, understand the inherent risks associated with Brazilian Jiu-Jitsu, agree to follow all academy safety rules and instructor directions, and, if completing this form on behalf of a participant under eighteen (18) years of age, certify that I am the participant's parent or legal guardian and have the legal authority to provide this consent.
